To help your visit run smoothly, we recommend completing your forms ahead of time.
If you’re unable to complete forms in advance, please plan to arrive a few minutes early and we’ll be happy to assist you.

To provide the most thoughtful and personalized care, we encourage you to send any relevant medical records prior to your visit.

Labs &
Insurance
While office visits and procedures are fee-for-service, we are able to submit lab work and vaccines to insurance on your behalf when applicable.
This allows you to still utilize your insurance benefits for those services while continuing to coordinate that portion of your care efficiently.

Cornerstone Family Wellness is a by-appointment practice only.
We are not a walk-in or urgent care clinic.
We do our best to accommodate patients when they are sick or need timely care, based on availability.
