Physician & Provider Collaboration

A Collaborative Approach to Patient Care

At Cornerstone Family Wellness, I deeply value the relationships patients have with their primary care providers and specialists.
This practice was not created to replace that care — but to come alongside it when patients need a more in-depth, personalized approach.
There are times in medicine when patients continue to struggle despite appropriate evaluation and treatment. Those are often the moments when a fresh perspective can be helpful.
That’s where I hope to serve as a resource.
When a Consultation May Be Helpful
I am available for consultation on patients who may feel “stuck” — those who have ongoing symptoms, complex histories, or concerns that haven’t yet found clear answers.
This may include:
- Persistent or unexplained symptoms
- Chronic conditions that are not responding as expected
- Patients seeking a more in-depth evaluation of lifestyle, nutrition, or underlying contributors
- Individuals who desire a functional or root-cause-oriented approach alongside their current care
My goal in these cases is simple:
To take a deeper look, ask different questions, and explore areas that may not have been fully addressed.
A Shared-Care Philosophy
I want to be very clear about one thing — I am not here to take over your patient relationships.
If you refer a patient to Cornerstone for consultation:
1
You remain their primary provider
2
I will focus specifically on the concerns for which they were referred
3
I am happy to communicate findings and recommendations back to you
4
Patients are always encouraged to continue their ongoing care with you
This is truly meant to be a collaborative effort, with the patient at the center.

Functional Medicine, Thoughtfully Applied
Functional medicine allows for a more comprehensive evaluation of how different systems in the body interact — including nutrition, lifestyle, environment, and underlying physiology.
This approach can be especially helpful in complex or chronic cases where standard approaches have not provided complete resolution.
My goal is to apply this thoughtfully and responsibly, always with respect for the care that has already been provided.

How To Connect
If you would like to discuss a patient, refer for consultation, or simply connect, I would welcome the conversation. You are also welcome to reach out to discuss whether a patient may be a good fit before referring.

A Shared Goal
At the end of the day, we all want the same thing —
patients who feel better, function better, and are truly heard.
If there is ever a way I can support that
goal for your patients, I would be honored to partner with you.
